Forever in our Hearts

Jake and his twin brother Brady came into this world Thursday evening, August 21st, 2014. Brady came first, but when Jake followed, he became entangled in his umbilical cord and was not breathing when he was born. The staff at Capital Health stabilized Jake and kept his body temperature low while the CHOP transport team arrived to bring him into the city.

Upon arrival at CHOP, Jake was started on therapeutic hypothermia where he was actively cooled to decrease inflammation of his brain and prevent further damage. After continued testing and monitoring it was determined that Jake had suffered severe hypoxic-ischemic encephalopathy (HIE) and his situation was described as "one of the worst they had ever seen" at CHOP.  Due to the severe nature of Jake's condition we quickly arranged for him to be baptized and he spent Friday evening with those who loved him: aunts, uncles, grandparents, and his parents. Jake's final moments on Earth were spent with his family. He passed away peacefully in the arms of his mommy early morning on Saturday, August 23rd 2014. Jake was with us for one day but has forever changed our lives.

We are setting up this page in memory of Jake with the hopes that we can advance research for HIE and therapeutic hypothermia and prevent these tragedies going forward.

Please donate in honor of Jake. We want to take his terrible tragedy and turn it into a positive impact on the world.

Jake, we will love you forever - Love Dad, Mom, Carter, Brady, and Riley

We have setup the Jake Freeman Memorial 5K as another avenue to raise funds for this cause. All proceeds from the race will be placed into this account.
